- Downgrade [POLICY] entry from INFO→DEBUG (redundant with caller logs)
- Add [DETAIL-POLICY-CHECK] debug log in detail() before policy check
- Enables distinguishing caller context: [DETAIL-POLICY] vs [LIST-PAGE-POLICY]
